Door to Door

Clay Transit is your transportation coordinator for Clay County. Clay Transit is a division of Council on Aging a private non- profit organization. We have been the Community Transportation Coordinator for Clay County since 1984. Any Clay County resident may request transportation services to obtain access to medical appointments, activities, we are the State of Florida Medicaid Non- Emergency provider and numerous of contracts.


Medicaid Transportation
Medicaid transportation is available to those Eligible beneficiaries who cannot obtain transportation on their own and meet criteria. An annual application must be completed to determine eligibility. Link to application documentation must be submitted in advance. A co-pay of $1 per one-way trip will be assessed. Appointments should be scheduled to go to the provider closest to your home.


Non-Sponsored Transportation
Non-sponsored services are available for individuals with no other means of transportation who may not own or cannot operate a personal vehicle. Riders will be assessed a copayment. All fees must be paid in full at time of service. Private-pay services are also available upon request. Riders may pay the full cost of trip. Call for a quote or for more information.
